(WKYT) -Ashley Ingram, a former payroll and HR manager, has filed a lawsuit against Madison Health and Rehabilitation Center for wrongful termination and retaliation.
According to the lawsuit, the facility was coming off a “special focus” status, and officials were concerned about seven days with no registered nursing coverage and one day with no director of nursing, which would earn it an unsatisfactory risk assessment from the Kentucky Cabinet for Health and Family Services.
To avoid another audit, facility officials allegedly came up with a plan to have a registered nurse falsely fill out a written time card showing that they worked shifts on seven days across Feb. and March, when there was no compliant nursing coverage before payroll and staffing information were submitted to the Centers for Medicare & Medicaid Services.
Ingram said was frequently asked to sign off on the allegdly fallsified time cards as being “accurate and true”, according to the lawsuit.
She refused to falsify the time cards and was met with “intolerable working conditions” in response, later resigning in April after which she filed a report with the state.
Ingram is now seeking monetary damages.
WKYT has reached out to Madison Health and Rehabilitation Center for comment but has not yet heard back.
